JoViPak® Order & Measuring Forms
The following order forms are available for ordering your JoViPak® product. If you are unsure as to which form to choose, please refer to the garment you are ordering on products page. All forms are in pdf format. A free Adobe Acrobat Viewer is necessary to open and print the pdf order forms. Please fax your order to us toll free.
New! Online Order Form for Ready-Made/Quick-Ship products
If you are ordering a Ready-Made product you can now use the online form and skip the fax machine. The form opens in Adobe Reader version 8 (free download), fill in your information on your computer, and then email it using a button on the form. Your order is instantly emailed to Jovipak. You can also keep a pdf copy of your order either digitally or in print.

To use this form, you must have Adobe Reader 8. (Free)
When the form opens, simply type in your order information. Certain area's are required. If you try to email only partial information, the form will prompt you to fill all required area's before it will email. If you would like to fax the form, simply press the "Print and Fax Button" on the form. The form will print out, and then you can manually fax the form. To save a digital copy, in the Adobe Reader toolbar choose "File > Save As". |
